XO Health

Launching a disruptor in an industry built on distrust.

Health insurance is one of the most distrusted industries in America. Launching a new player into that landscape — one promising to actually do things differently — requires more than a compelling product. It requires a brand system capable of holding two almost contradictory things at once: authority to make patients feel safe, and warmth to make them feel seen.

XO Health

The Problem

XO Health wasn’t just entering a crowded market. It was asking people to trust a new name with something they’d already been burned by.

XO Health is a health insurance disruptor; designed by employers, engineered by experts, and built on the conviction that equitable, sustainable healthcare is achievable. Seed-funded by AlleyCorp, XO came to Macroscopic ahead of launch needing a brand identity and communication system ready to perform across two very different audiences: the self-insured employers evaluating XO as a business decision, and the plan members who would actually live inside it.

The challenge wasn’t differentiation for its own sake. It was earning trust in a sector where trust has been systematically eroded while doing it fast enough to support launch timelines.

The Bigger Pattern

The insight that shaped this engagement, that the same message can build confidence in one audience and create distance in another, is easy to miss when you’re deep inside a launch. It takes outside perspective to see where your brand is working against you. Our job is to find those fractures early, and design systems resilient enough to hold across every audience, every channel, every moment of contact.

An evolving brand ecosystem

Brand Identity & Strategic Positioning

We began by getting close to the business. We worked directly with XO’s founder as she was actively pitching investors and prospects, absorbing real-time market feedback and building the brand in dialogue with it. We also launched the inaugural XO Health Design Studio: a symposium bringing together stakeholders from across the health plan lifecycle to pressure-test the vision and surface what would actually resonate.

What came out of that process wasn’t just a visual identity. It was a positioning framework that could hold its own against legacy incumbents. It conveyed the strength, expertise, and operational rigor that employers needed to see, while signaling something genuinely new.

Member Brand: A Critical Pivot

Early in the work, we surfaced an insight that changed the direction of the member-facing brand: the same strength-centered language that built employer confidence was a deterrent for plan members navigating health crises. People in moments of stress and vulnerability don’t need to be empowered. They need to feel held.

That led us to develop a distinct member sub-brand. One that carried XO’s core values of clarity, confidence, and ease, but translated them into something more human and advocacy-led. The visual and tonal language shifted without breaking coherence with the primary brand; a design challenge that required precise calibration at every touchpoint.

Operational Brand Infrastructure

A brand that only lives in a style guide doesn’t scale. We delivered a comprehensive Member Brand Experience Guidelines document: an operational framework giving XO’s team the tools to execute brand stewardship consistently as the organization grows, without needing to reinvent decisions that had already been made carefully.

What it’s unlocked

  • Insurance plan launched, actively accruing self-insured business clients
  • Improved trust and affinity among plan members, particularly those in stress and crisis
  • Additional funding rounds secured, with open market launch on track
  • Scalable brand infrastructure in place to support continued growth
